Consent Form

Once a purpose is created, it can be linked to a consent form. Consent forms are created using a guided, multi-step wizard.

Creating Consent Form

Accessing the Consent Form Wizard:

  1. From the left navigation pane, go to Consent Management > Consent Forms.
  2. Click Add Consent Form to launch the wizard.
    Wizard Steps:
    Step 1: Details

    1. Enter the Consent Form Name, Description, Organizational Unit, and Language.
    2. If you want to add Indian languages, click Add all Indian Languages in Quick Selections section.
    3. Note:

      • Support is available for 22 Indian regional languages.
      • Selected languages will be visible on the consent form in drop down menu for users to choose from.
    4. Click Next to proceed.

    Step 2: Statements

    1. Add one or more Consent Statements.
    2. Choose the input type for each statement (e.g., radio buttons, checkboxes).
    3. Preview the statements in the Preview section.

    Step 3: Design

    1. Select a color scheme from existing options or create a new one.
    2. Use the Preview section to see real-time updates.
    3. Customize branding elements:
      • Logo and its alignment:
      • Fonts
      • Header and Footer

    Step 4: Language Preview

    • Choose the languages in which the consent form should be available

Following actions can be performed on the consent form:

  • Edit

    While editing the published consent form in the Statements section, if you update the Display Statement and need to request the user’s consent again, select Breaking Change. If the update does not require re-submission of user’s consent, select Non-Breaking Change.

Integrating Consent Form with Your Website

To integrate the consent form, follow these steps:

  1. Once the SDP Admin creates consent form, download it. A ZIP folder containing HTML, CSS and
    Javascript is downloaded.

  2. Provide it to Application developer.
  3. Application developer should integrate the HTML in the UI screen where consent form needs to appear.
  4. In the HTML, the DataPrincipalId Key can be hardcoded. The DataPrincipalId Value need to be assigned dynamically for the end user who is providing the consent.
