Consent Form

Once a purpose is created, it can be linked to a consent form. Consent forms are created using a guided, multi-step wizard.
Accessing the Consent Form Wizard:

  1. From the left navigation pane, go to Consent Management > Consent Forms.
  2. Click Add Consent Form to launch the wizard.
    Wizard Steps:
    Step 1: Details
    • Enter the Consent Form Name, Description, Organizational Unit, and Language.
    • Click Next to proceed.
    Step 2: Statements
    • Add one or more Consent Statements.
    • Choose the input type for each statement (e.g., radio buttons, checkboxes).
    • Preview the statements in the Preview section.
    Step 3: Design
    • Select a color scheme from existing options or create a new one.
    • Use the Preview section to see real-time updates.
    • Customize branding elements:
    o Logo and its alignment
    o Fonts
    o Header and Footer
    Step 4: Language Preview
    • Choose the languages in which the consent form should be available.
