Creating a Consent Form

Accessing the Consent Form Wizard

  1. From the left navigation pane, go to Consent Management > Consent Forms.
  2. Click Add Consent Form to launch the wizard.

Step 1: Details

  1. Enter the Consent Form Name, Description, Organizational Unit, and Language.
  2. If you want to add Indian languages, click Add all Indian Languages in Quick Selections section.
  3. Note:

    Support is available for 22 Indian regional languages.
    Selected languages will be visible on the consent form in drop down menu for users to choose from.

  4. Click Next to proceed.

Step 2: Statement: Configuring the Consent Statement

  1. Add one or more Consent Statements.
  2. Choose the input type for each statement (e.g., radio buttons, checkboxes).
  3. Select one or more purposes from the dropdown list.
    Configure rejection/withdrawal options.

You can mark a specific option as a Rejection/Withdrawal option within a consent form. This helps the system automatically identify whether a data principal has accepted, rejected, or withdrawn their consent.

Configure Rejection and Withdrawal Behavior

  • Depending on the type of consent you are creating, select appropriate Option Selection Type:
    • Radio Button
    • Dropdown
    • Checkbox

Configure Rejection/Withdrawal Options for Radio Button:

  1. Add the required options.
  2. Turn on the Mark Rejection (Withdrawal) toggle for the option that should represent rejection.
  3. (Optional) Select Non-selection of any option will be treated as rejection if you want no selection to be considered a rejection.

How it Works?

  • First-time consent submission
    • If the data principal selects an option marked as Rejection/Withdrawal, the consent is recorded as rejected.
    • Non selection of any option by data principal is recorded as rejected.
    • If the data principal selects any other option, the consent is recorded as accepted.
  • Preference Center

    • If the data principal changes their selection from an accepted option to an option marked as Rejection/Withdrawal , the system records the event as a withdrawal and marks the consent as rejected.
    • If the data principal changes their selection from an accepted option, the system records the event as withdrawal.

Note: You can mark a maximum of N-1 options as Rejection/Withdrawal. The last remaining option cannot be marked, and its toggle is automatically disabled.

Configure Rejection/Withdrawal Options for Checkbox:

  1. Add the required option.
  2. Select Non-selection of any option will be treated as rejection if you want no selection to be considered a rejection.

Note: You cannot mark any checkbox option as a Rejection/Withdrawal option.

How it Works?

  • First-time consent submission
    • If the data principal selects the checkbox, the consent is recorded as accepted.
    • If the data principal does not select the checkbox, the consent is recorded as rejected.
  • Preference Center
    • If the checkbox was previously selected and the data principal clears it, the system records the event as a withdrawal and marks the consent as rejected.

Configure Rejection/Withdrawal Options for Dropdowns
Dropdown behavior depends on whether the field allows single selection or multiple selections.

Single-select dropdown

  1. Add the required options.
  2. Turn on the Mark Rejection (Withdrawal) toggle for the option that should represent rejection or withdrawal.

How it Works?

  • First-time consent submission
    • If the data principal selects an option marked as Rejection/Withdrawal, the consent is recorded as rejected .
    • If the data principal selects any other option, the consent is recorded as accepted.
  • Preference Center
    • If the data principal changes the selected value to an option marked as Rejection/Withdrawal, the system records the event as a withdrawal and marks the consent as rejected .

Note: You can mark a maximum of N-1 options as Rejection/Withdrawal. The last remaining option cannot be marked, and its toggle is automatically disabled.

Multi-select dropdown

  1. Add the required option.
  2. Select Non-selection of any option will be treated as rejection if you want no selection to be considered a rejection.
  3. Note: You cannot mark any option as a Rejection/Withdrawal option.

Preview the statements in the Preview section.

Step 3: Design

  1. Select a color scheme from existing options or create a new one.
  2. Use the Preview section to see real-time updates.
  3. Customize branding elements:
    • Logo and its alignment
    • Fonts
    • Header and Footer

Step 4: Language Preview

  • Choose the languages in which the consent form should be available
